Mosaic is looking for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team!

In this role, you will provide health care to individuals receiving services in their homes by scheduling medical appointments, assessing their condition, reviewing medications, completing necessary medical forms, and updating medical records.

What YOU'LL do:

• Coordinate physician and health care services for people served
• Conduct health care assessments and write health care plans
• Facilitate training and orientation of new staff regarding healthcare procedures
• Provide on-call assistance to staff as needed

SCHEDULE: Days (7a-3p) 40 hrs/week, rotating weekends / Evenings (3-11pm) 40 hrs/week, rotating weekends / Part-Time Nights (11pm-7am) 20 hrs/week, rotating weekends.
