Emerald Nursing and Rehab Lakeview in Grand Island, NE is seeking an experienced Licensed Practical Nurse (LPN) to join our Nursing team. You will provide direct patient care, administer medications, perform wound care, and monitor residents under the supervision of RNs and the DON.

Shift: Full-Time 6pm-6am. LPN/LVN license valid for NE or multi-state, CPR/BLS required. Strong communication and teamwork skills essential for coordinated care in a skilled nursing facility.

Competenze

• Licensure: Valid LPN/LVN license in Nebraska or Multi-state.
• Education: Graduation from an accredited LPN/LVN program.
• Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.
• Certifications: Current CPR/BLS certification required.
• Communications: Excellent verbal and written communication skills.
• Physical Requirements: Ability to lift and move patients and stand for extended periods.

Responsibilities

• Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.

• Monitoring and Assessment: Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.

• Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.

• Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.

• Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.

• Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.

• Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Qualifications

• Licensure: Valid LPN/LVN license in Nebraska or Multi-state

• Education: Graduation from an accredited LPN/LVN program.

• Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.

• Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
