Job summary

Plan A Health in Baltimore, MD is seeking a licensed practical nurse (LPN) to provide resident-centered nursing care in a specialized long-term care unit for pulmonary and complex medical conditions.

Under supervision, the LPN will develop and implement individualized care plans, administer medications, monitor and document resident conditions, collaborate with the interdisciplinary team, educate residents and families about care goals, and support daily unit operations including shift

Qualifications

• Maryland LPN license current and CPR certification.
• Graduation from an accredited nursing program.

Job description

We are seeking a Licensed Practical Nurse (LPN) to provide resident-centered nursing care in a specialized long-term care unit dedicated to pulmonary and complex medical conditions.

Under supervision, the LPN will

• develop and implement individualized care plans,
• administer medications and treatments according to physician orders,
• monitor and document resident conditions,
• collaborate with interdisciplinary team members,
• educate residents and families about care goals,

and

• support daily unit operations including shift leadership as needed.

Qualifications include a current Maryland LPN license, CPR certification, and graduation from an accredited nursing program.

Preferred experience includes

• long-term care, pulmonary, rehabilitation, or tracheostomy care,

with strong skills in

• communication, critical thinking,

and

• organization.

Proficiency with electronic health records, IV pumps, monitoring systems, and specialty beds is desired.
