Job summary

Catholic Health Home Care in New York is seeking passionate LPNs to provide comprehensive home health care to patients in their homes across communities such as Bohemia, Brentwood, Fire Island, Great River, Islandia, Islip, Lake Ronkonkoma, Oakdale and West Sayville.

You will assess, plan and implement care, monitor vitals, administer IVs, manage wounds, and educate patients and families, collaborating with physicians, therapy and social services to ensure quality, compassionate care.

Qualifications

• New York State LPN license is required.
• Minimum of one year of hands-on nursing experience.
• Basic Life Support (BLS) certification is required.

Responsibilities

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for patients.
• Measure vital signs, administer IVs, change dressings, clean wounds as needed.
• Provide skilled nursing care and rehabilitative procedures within diverse home settings.
• Collaborate with physicians, therapy, social services and home health aides for your caseload.
• Educate patients and families on disease management and health promotion.

• EVERY Weekend Shift - 7:00AM to 5:30PM ( 10 Hr Shift)

Job Details

Our Home Care LPN’s

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.

Minimum Qualifications

• New York State Licensed Practical Nurse (LPN) license
• Minimum of one year of recent hands on experience
• Basic Life Support Certification

Salary Range

USD $35.00 - USD $45.00 /Hr.
