Job summary

Catholic Health Home Care is seeking compassionate LPNs to deliver comprehensive home health care to patients across the assigned caseload. You will perform assessments, manage care plans, and collaborate with a multidisciplinary team.

Weekend Day shift: 7:00AM to 5:30PM. Responsibilities include measuring vital signs, administering IVs, wound care, and patient education to support disease management and health promotion.

Qualifications

• New York State Licensed Practical Nurse (LPN) license is required.
• Minimum of one year of recent hands-on nursing experience.
• Basic Life Support Certification (BLS) required.

Responsibilities

• Case manage, assess, plan, develop and implement a home health care plan.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed.
• Provide skilled nursing care and prescribed treatments for diverse home care patients.
• Collaborate with physicians, therapy, social services and other team members.
• Educate patients and families on disease management and health promotion as directed.

Overview

Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

Team 7: Belle Terre, Centereach, Farmingville, Head of the Harbor, Lake Grove, Nesconset, Nissequogue, Old Field, Port Jefferson, Port Jeff Station, Setauket, St. James, Stonybrook, & Terryville.

• Weekend Day shift: 7:00AM to 5:30PM
• Provide 16-18 visits per weekend- ensuring coverage of high priority cases

Job Details

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Minimum Qualifications:

  • Minimum of one year of recent hands on experience
  • Basic Life Support Certification

Salary Range

USD $33.00 - USD $40.00 /Hr.
