Job summary

Catholic Health Home Care is seeking licensed practical nurses to deliver comprehensive in-home care across multiple NY service areas.

As an LPN you will case manage, assess, plan, and implement care plans, measure vitals, change dressings, and provide wound care while coordinating with a multidisciplinary team.

Shifts are 5x8 hours (8:30 AM–4:30 PM) with rotating weekends; benefits include health coverage and education reimbursement, plus additional compensation for heavy caseloads.

Qualifications

• New York State LPN license required.
• Minimum of one year of hands-on experience.
• Basic Life Support Certification (BLS) required.

Responsibilities

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for patients.
• Measure vital signs, administer IVs, change dressings, clean wounds as needed.
• Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with diverse patient populations in home settings.
• Collaborate with physicians, therapy, social services and home health aides for the assigned caseload.
• Educate patients and families on disease management and health promotion.

Overview

Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

Team 8: Coram, Middle Island, Miller Place, Mount Sinai, Ridge, Rockypoint, Selden, Shoreham, Sound Beach, Upton, Wading River, Yaphank, North Fork , Aquebogue, Baiting Hollow, Calverton, Flanders, Jamesport, Laurel, Mattituck, Northville, & Riverhead.

• 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details

Our Home Care LPN's:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.


• Measure vital signs, Administer IVs, change dressings, clean wounds as needed


• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.


• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.


• Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.

Minimum Qualifications:

• New York State Licensed Practical Nurse (LPN) license


• Minimum of one year of recent hands on experience


• Basic Life Support Certification

Salary Range


USD $33.00 - USD $40.00 /Hr.
