Job summary

Hartford HealthCare in Stratford, CT is seeking a flexible LPN to join our regional care team. You will travel to regional urgent care sites as needed and provide direct patient care under RN or Provider supervision.

You will collaborate with care teams to document patient data, support care plans, and perform therapeutic measures to help patients achieve their health goals.

Qualifications

• Current Connecticut LPN license is required.
• CPR certification is required.
• Medical/surgical office experience preferred.

Responsibilities

• Collaborates with members of health care team to assist patients in achieving established outcomes of care.
• Obtains, documents and reports patient data related to the established plan of care.
• Performs therapeutic measures under the direction of an RN or Provider to assist in achieving established patient care outcomes.

Job Summary
