Job summary

Hartford HealthCare is seeking a Licensed Practical Nurse to provide quality, cost-effective direct care for patients within a unit or service. Under the supervision of a Registered Nurse or Provider, you will collaborate with the health care team to meet planned outcomes.

You will participate in direct patient care, collect and report data, and perform therapeutic measures under RN direction to support patient goals. Join us in a network committed to growth and community health.

Qualifications

• CT LPN license required.
• CPR certification required.
• Medical/Surgical office experience preferred.
• Specialization and/or specific work experience may be required.

Responsibilities

• Collaborates with health care team to assist patients in achieving outcomes of care.
• Assists patients by obtaining, documenting and reporting data related to the established plan of care.
• Performs therapeutic measures under the direction of an RN or Provider to support patient outcomes.

Job Summary

Under the supervision of a Registered Nurse, or Provider, the LPN provides quality and cost-effective direct care for patients/families within a unit, service or other primary area of practice.

• Collaborates with members of health care team to assist patients in achieving established outcomes of care
• Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
• Performs therapeutic measures, under the direction of an RN or Provider, to assist in achieving established patient care outcomes.

• Current Connecticut Licensed Practical Nurse (LPN) license required
• CPR certification
• Medical/Surgical office experience preferred
