Job summary

Hartford HealthCare is seeking a per diem Licensed Practical Nurse to provide high-quality, cost-effective direct patient care across units. You will float to regional Urgent Care sites, collaborating with multiple teams to support patient outcomes.

Under RN or Provider supervision, you will document patient data and perform therapeutic measures to achieve care goals, aligning with established plans of care.

Qualifications

• Current Connecticut LPN license required.
• CPR certification required.
• Urgent Care experience preferred.
• Specialization and/or specific work experience may be required.

Responsibilities

Job Summary

Under the supervision of a Registered Nurse, or Provider, the LPN provides quality and cost-effective direct care for patients/families within a unit, service or other primary area of practice. LPN will float to regional Urgent Care sites bringing variety to your workday and the chance to collaborate across multiple teams and communities.

• Collaborates with members of health care team to assist patients in achieving established outcomes of care
• Participates in direct patient care in order to meet individual and family health goals by obtaining, documenting and reporting patient data related to established plan of care
• Performs therapeutic measures, under the direction of an RN or Provider, to assist in achieving established patient care outcomes.

Qualifications
