Job summary

RiverSpring Living is seeking an LPN Supervisor for an immediate per diem opportunity at our Riverdale, NY location. The role leads a team of Home Health Aides, delivering nursing care and ensuring adherence to care plans under RN/DON supervision.

The LPN must be a graduate of an accredited program with a NY LPN license and 1–2 years of community or adult care experience; supervisory experience and medication management are preferred. Compensation is $40.00 per hour.

Qualifications

• Graduate of an accredited practical nursing program.
• Valid New York State License as an LPN (NY).
• Minimum of 1–2 years of experience in community nursing, acute care, or adult care facilities.

Responsibilities

• Supervises Home Health Aides and reinforces residents’ care plans.
• Delivers nursing care following agency policies and procedures.
• Assists with resident admissions and conducts daily stand-up meetings with HHAs.
• Maintains accurate clinical records and prepares oral reports.
• Assists in preparing and evaluating nursing care plans and documents changes in residents' conditions.
• Manages medications, including changes, new prescriptions, and refills.
• Supports nursing management with assessments and reassessments of residents.

RiverSpring Living is seeking a Licensed Practical Nurse (LPN) Supervisor to join our dynamic team of professionals at our RiverSpring Licensed Home Care Services Agency!

The RiverSpring Licensed Home Care Services Agency (LHCSA) provides comprehensive and personalized home healthcare services for older adults to maximize their independence at home. The LPN Supervisor will provide neighborhood leadership and essential nursing services to residents and their families within our Assisted Living Residence (ALR) in accordance with the agency’s policies and procedures under the supervision of the Registered Nurse and the Director of Nursing of the ALR.

The RiverSpring Living campus is set on 32 acres overlooking the Hudson River, in Riverdale, New York.RiverSpring Living provides a full range of care solutions including nursing home care, managed long term care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care.

Key Responsibilities:

Supervisory

• Oversees neighborhoods and manages Home Health Aides (HHAs) in reinforcing elements of residents’ plan of care
• Provide supervision and direction to Home Health Aides

Operations

• Deliver nursing care in accordance with agency policies and procedures.
• Participate in incident reporting and follow-up actions.
• Assist with resident admissions and conduct daily stand-up meetings with Home Health Aides.
• Completes, maintains and submits accurate clinical records and prepares oral reports.

Clinical

• Assist in preparing and evaluating nursing care plans.
• Observe, document, and report changes in residents’ conditions
• Perform nursing procedures, including medication administration and treatments.
• Manage medication for residents, including changes, new prescriptions, and refills.
• Assists Nursing management with the assessments and reassessments of residents to determine health needs.

• Addresses employee concerns and ensures that staff adhere to organizational policies.

Residents Relations & Support

• Ensures a safe and healthy living environment for all residents.

Qualifications/Skills:

• Graduate of an accredited practical nursing program.
• Valid New York State License as a Licensed Practical Nurse (LPN).

Compensation:
