Avamere Rehab of Coos Bay in Coos Bay, OR is seeking a Licensed Practical Nurse (LPN) to provide day-to-day nursing care and support to residents. The role involves vital signs, medication administration, wound care, and assisting with daily activities under the supervision of an RN.

This PRN position requires clinical competence in basic nursing care, documentation, and collaboration with the care team to ensure resident well-being and safety.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active LPN license in this state.
• Must maintain and have an active CPR/BLS during employment.
• Experience in long term care or geriatric rehab settings preferred.
• Experience in electronic medical records documentation.

Responsibilities

• Provide basic nursing care: vitals, med administration, wound care, hygiene and mobility, nutrition and hydration.
• Record patient history and monitor current condition; report changes to RN.
• Work under RN supervision; supervise CNAs; collaborate with other health professionals to ensure comprehensive care.
• Document all care provided, including medications, vital signs, and observations; maintain patient safety.
• Offer emotional support to residents and families; follow infection control protocols and safety guidelines.

Job Summary

Licensed Practical Nurse provides basic nursing care, including vital signs, medication administration, wound care, and assisting residents with daily activities, under the supervision of a Registered Nurse (RN)

Essential Duties And Job Responsibilities

Basic Nursing Care

• Vital Signs: Accurately measure and record vital signs like temperature, pulse, blood pressure, and respirations.
• Medication Administration: Administer medications as prescribed by a physician, following facility protocols and under the supervision of an RN.
• Wound Care: Perform wound care, including changing dressings and monitoring for signs of infection.
• Hygiene and Mobility: Assist residents with personal hygiene, dressing, bathing, and mobility.
• Nutrition and Hydration: Ensure residents receive proper nutrition and hydration, assisting with feeding if necessary.

Patient Monitoring

• Patient History: Record a patient's history and current condition.
• Observation: Observe and document changes in a resident's condition, reporting any concerns to the RN.
• Communication: Communicate effectively with residents, families, and other healthcare professionals

Collaboration and Supervision

• RN Supervision: Work under the supervision of an RN, following their guidance and instructions.
• CNA Supervision: Supervise certified nursing assistants (CNAs) in providing basic nursing care.
• Teamwork: Collaborate with other healthcare professionals to ensure residents receive comprehensive care.
• Other Duties:
• Patient Safety: Maintain a safe environment for residents, following infection control protocols and other safety guidelines.
• Documentation: Accurately document all care provided, including medications administered, vital signs, and any observations.
• Emotional Support: Provide emotional support and comfort to residents and their families.

Requirements

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an LPN in this state
• Must maintain and have an active CPR/BLS during employment
• Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting
• Experience in electronic medical records documentation.
