Job summary

Human Touch Home Health is seeking compassionate Licensed Practical Nurses (LPNs) to provide skilled home health nursing in Silver Spring and surrounding Montgomery County communities. Multiple schedules available including Full-Time, Part-Time, and PRN.

Full-Time requires coverage throughout Montgomery County; Part-Time/PRN offer flexible territory options. Competitive compensation and benefits are provided where eligible.

Qualifications

• Active Maryland LPN license is required.
• Graduate of an approved Practical Nursing program.
• Reliable transportation to multiple territory locations.
• Strong clinical assessment and communication skills.
• Excellent time-management and organizational abilities.
• Ability to work independently in patients' homes.
• Full-Time candidates must be able to cover all of Montgomery County.
• WellSky experience is a plus.

Responsibilities

• Provide skilled nursing visits in patients' homes.
• Perform focused nursing assessments.
• Monitor vital signs and patient condition changes.
• Provide wound care and treatments as ordered.
• Assist with medication management and education.
• Reinforce disease-management education.
• Monitor treatment effectiveness and patient response.
• Educate patients and families.
• Communicate changes in condition to the care team.
• Follow physician orders and plans of care.
• Collaborate with the home health team.
• Document accurately using WellSky.

What You'll Do

Working under the direction of the RN and physician-established plan of care, you will:

• Provide skilled nursing visits in patients' homes

• Perform focused nursing assessments

• Monitor vital signs and changes in patient condition

• Provide wound care and treatments as ordered

• Assist with medication management and medication education

• Reinforce disease-management education

• Monitor treatment effectiveness and patient response

• Educate patients, families, and caregivers

• Communicate changes in condition promptly to the RN and appropriate members of the care team

• Follow physician orders and established plans of care

• Collaborate with the interdisciplinary home health team

• Complete accurate and timely clinical documentation using WellSky

What We're Looking For

• Active Maryland LPN License Required

• Graduate of an approved Practical Nursing program

• Reliable transportation

• Strong clinical assessment and communication skills

• Excellent time-management and organizational abilities

• Ability to work independently in patients' homes

• Full-Time candidates must be able to cover all of Montgomery County

• Home Health Experience Preferred, but Not Required
