Job summary
Human Touch Home Health is seeking compassionate Licensed Practical Nurses (LPNs) to provide skilled home health nursing in Silver Spring and surrounding Montgomery County communities.
We offer Full-Time, Part-Time and PRN opportunities with flexible territory options. Full-Time covers Montgomery County; Part-Time/PRN allow selection of preferred communities. Join a employee-focused team delivering one-on-one patient care.
Qualifications
- Active Maryland LPN License Required.
- Graduate of an approved Practical Nursing program.
- Reliable transportation.
- Strong clinical assessment and communication skills.
- Excellent time-management and organizational abilities.
- Ability to work independently in patients' homes.
- Full-Time candidates must be able to cover all of Montgomery County.
- WellSky experience is a plus.
Responsibilities
- Provide skilled nursing visits in patients' homes.
- Perform focused nursing assessments.
- Monitor vital signs and changes in patient condition.
- Provide wound care and treatments as ordered.
- Assist with medication management and education.
- Reinforce disease-management education.
- Monitor treatment effectiveness and patient response.
- Educate patients, families, and caregivers.
- Communicate changes in condition promptly to the RN and care team.
- Follow physician orders and plans of care.
- Collaborate with the home health team.
- Complete accurate and timely clinical documentation using WellSky.
