Job Description

AHRC NYC is looking for Full and Part-Time Licensed Practical Nurses (LPN) for our ICF residences in Manhattan and Queens. The LPN will monitor health and safety needs of people supported living in our residences to promote wellness.

Manhattan: 126 East 126th Street Manhattan

Full-Time: 3 days/week... Mon/Wed/Fri 7am-7pm or 7pm-7am... $62,400 per year plus comprehensive health benefit package. See additional Benefit information below. Part-Time: Saturday overnight 7pm-7am... $33 per hour

Queens: 89-02 162nd Street in Jamaica.

Full-time: Overnight Friday - Sunday night... 7pm-7am... $62,400 per year plus benefit package. Part-time: 7am-7pm... Monday... $33 per hour.

ESSENTIAL RESPONSIBILITIES: (include but are not limited to):

  1. Supervise and participate with the direct support staff tasks related to personal hygiene, Activities of Daily Living, incontinence care, and environmental cleanliness, ensuring these tasks are properly performed.
  2. Supervise and participate with the direct support staff in mealtime and feeding protocols, to assure adherence to recommended nutritional and medical guidelines.
  3. Perform medication administration as per procedure outline.
  4. Report and oversee infection control practices as outlined per agency and per program-specific exposure control plans.
  5. Oversee and ensure adequate basic First Aid supplies are available as necessary for the safety of people supported, per departmental guidelines.
  6. Report and document medical findings for RN review and for inclusion to a person’s plan of nursing services.
  7. Reporting immediately any significant deviations in the medical status of a person supported to RN Supervisor.
  8. Communicate, report and document the issues and concerns of the person supported to primary medical provider and RN Supervisor.
  9. Communicate with families and maintain family contacts to inform and discuss pertinent medical concerns of people supported.
  10. Provide post-operative, routine or preventative medical treatments and interventions as per MD or RN order (i.e., G-tube feedings, wound and skin care, urinary drainage systems maintenance, blood glucose monitoring, passive and active range of motion therapy).
  11. Obtain baseline and monitor medical information (Vital Sign Monitoring, Weights).
  12. Ensure ongoing compliance and quality assurance with Federal and State regulations pertinent to the operation of the residence.
  13. Utilize appropriate and expected medical charting skills.
  14. Attend medical appointments as requested by RN Supervisor as individual LPN scheduling permits.
  15. Observe and follow through on established emergency notification procedures in a manner which will ensure the safety and welfare of all people supported.
  16. Provide coverage in accordance with established staffing schedules.
  17. Attend and successfully complete all necessary training programs.
  18. Perform other duties as assigned.
