Job summary

Lifepoint Health's TGH Rehabilitation Hospital in Tampa, FL is seeking a Licensed Practical Nurse (LPN) for a full-time night shift in rehabilitation. You will apply nursing assessments, planning, implementation and evaluation of care to help patients regain independence.

Collaborate with a multidisciplinary team, engage families, monitor patient conditions, and support safe discharges. Requires LPN licensure in Florida, CPR/BCLS, with 6 months Med/Surg experience preferred; ACLS preferred.

Qualifications

• Graduation from an accredited nursing program (BSN/ADN or Diploma).
• Current state licensure as Licensed Practical Nurse.

Responsibilities

• Deliver direct and indirect patient care using the nursing process (assessment, planning, intervention, evaluation).
• Collaborate with patients, families, and the interdisciplinary team to create care plans.
• Monitor and communicate changes in patient conditions to physicians and team members.
• Contribute to discharge planning to ensure safe, smooth transitions.

How You'll Make An Impact

As a Licensed Practical Nurse, you'll be a trusted partner in our patients' recovery journeys. You will:

• Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation
• Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues
• Play a key role in the discharge planning process to support safe, successful transitions

Qualifications And Requirements

• Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.

Additional Requirements Include

• CPR/BCLS certification. ACLS preferred.
• Minimum six months' Medical/Surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.
