Job summary

TGH Rehabilitation Hospital is recruiting a Licensed Practical Nurse (LPN) for night shifts in our rehabilitation unit. You will provide direct and indirect patient care, collaborate with an interdisciplinary team, and support discharge planning to ensure smooth transitions for patients.

Qualifications

• Graduate of an accredited LPN program with current state license.
• CPR/BCLS certification; ACLS preferred.
• Minimum six months of Medical/Surgical experience in an acute care setting.

Responsibilities

• Deliver direct and indirect patient care using the nursing process (assessment, planning, intervention, evaluation).
• Collaborate with patients, families, and the interdisciplinary care team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians and supervisors.
• Participate in discharge planning to support safe transitions.

Qualifications and requirements:

• Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.

Additional requirements include:

• CPR/BCLS certification. ACLS preferred.
• Minimum six months' Medical/Surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.
