Job summary

ClearSky Health is seeking a dedicated Licensed Practical Nurse (LPN) for our Lancaster, Ohio location. As part of the compassionate care team, the LPN will deliver skilled nursing services under the supervision of a Registered Nurse, support individualized care plans, and ensure a safe environment for patients.

About the Role

As a Licensed Practical Nurse (LPN), you will be a key contributor to our patient care team, delivering skilled nursing services under the supervision of a Registered Nurse and in alignment with physician orders. Your role is essential in supporting individualized care plans, administering treatments, and ensuring a safe and healing environment for every patient. You’ll collaborate closely with RNs, physicians, and other healthcare professionals to provide responsive, high-quality care.

Position Highlights at a Glance

• Caseload: ~7 patients
• Average patient length of stay: 10–14 days
• Conditions: Stroke, brain & spinal cord injuries, cardiac, ortho, complex medical, and more
• Team approach: work closely with PT/OT/ST, physicians, case managers, respiratory therapy & more
• Advanced care: IVs, trachs, wound care, mobility support, lines, tubes, and drains
• 12‑hour shifts, 3 days/week + rotating weekends/holidays (with added pay)

What You’ll Need

• LPN License (State of Ohio)
• CPR required | ACLS & BLS preferred
• 1+ year of LPN experience preferred (acute care a plus)

Physical Requirements

• Significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time
• Lifting/exerting of up to 50 lbs.
• Possible exposure to bodily fluids
• Visual acuity required for patient assessment and documentation of care
• Acute hearing required for accurate patient assessment
• Sufficient manual dexterity to operate equipment and computer keyboard
• Close vision and the ability to adjust focus
