Job summary

ClearSky Health in Wisconsin is seeking a Licensed Practical Nurse (LPN) to join our inpatient care team, delivering skilled nursing under RN supervision and physician orders. You will support individualized care plans, administer treatments, manage IVs and wounds as needed, and collaborate with RNs, physicians, case managers, and therapists to ensure safe, high‑quality patient care.

This role offers 12‑hour shifts (3 days/week) with rotating weekends/holidays, competitive pay, a signing bonus

Qualifications

• LPN License (State of WI) required.
• CPR required; ACLS & BLS preferred.
• 1+ year LPN experience preferred (Acute care a plus).

About the Role

As a Licensed Practical Nurse (LPN), you’ll be a key contributor to our patient care team, delivering skilled nursing services under the supervision of a Registered Nurse and in alignment with physician orders. Your role is essential in supporting individualized care plans, administering treatments, and ensuring a safe and healing environment for every patient. Position Highlights at a Glance

• Caseload: ~7 patients
• Average Patient Length of Stay: 10-14 days
• Conditions: Stroke, brain & spinal cord injuries, cardiac, ortho, complex medical, and more
• Team Approach: Work closely with PT/OT/ST, physicians, case managers, respiratory therapy & more
• Advanced Care: IVs, trachs, wound care, mobility support, lines, tubes, and drains
• 12-hour shifts, 3 days/week + rotating weekends/holidays (with added pay)

What You’ll Need

• LPN License (State of WI)
• CPR required | ACLS & BLS preferred
• 1+ year of LPN experience preferred (Acute care a plus)

Physical Requirements Over the Course of a Shift

• A significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time.
• Both gross and precise motor functions.
• Lifting/exerting of up to 50 lbs.
• Possible exposure to bodily fluids.
• Visual acuity required for patient assessment and documentation of care.
• Acute hearing required for accurate patient assessment.
• Sufficient manual dexterity to operate equipment and computer keyboard.
• Close vision and the ability to adjust focus.
