Job summary

Heritage Woods of Greenbriar in Indianapolis, IN is seeking a licensed practical nurse (LPN) to oversee medication distribution in accordance with physician orders and our medication management procedures. You will support resident safety, independence, and dignity.

You will develop personalized care plans, coordinate with physicians and family, participate in quality assurance initiatives, and pursue ongoing learning through seminars and staff meetings.

Qualifications

• Licensed Practical Nurse (LPN) license required.
• CPR certification preferred/required.
• Demonstrated compassion and strong communication with residents and families.

Responsibilities

• Oversees medication distribution per physician orders and in line with medication management procedures.
• Develops personalized service plans tailored to individual care needs.
• Communicates with physicians, other healthcare providers, and family about residents’ physical, mental, and social well‑being.
• Contributes to quality assurance and performance improvement initiatives.
• Engages in ongoing learning through seminars, staff meetings, and other training.
