The Licensed Practical Nurse (LPN) in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.

Responsibilities

• Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. May not alter the plan of care.
• Instructs the patient's family in care and maintaining a healthy environment.
• Observes, records, and reports the patient's response to treatment and any changes in condition to the supervising nurse and/or physician daily.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
• CPR Certification required.
