Job summary

IMAGE STRATEGIC PARTNERS seeks an LPN to provide direct patient care for geriatric residents. You will follow physician orders, document histories, and monitor vital signs and patient progress in a community setting.

Responsibilities include injections, wound care, specimen collection, and family communication. A current LPN license and background check are required; 1 year of relevant experience is preferred for this role.

Qualifications

• Must have a current state LPN license.
• 1 year of experience as an LPN in a community-based care setting preferred.
• Comply with state regulations and company policies.
• Pass criminal background check.
• LPN students meeting criteria may be considered.

Responsibilities

• Follow physician orders and provide detailed assessment data for quality resident care.
• Record a patient’s medical history accurately.
• Take and record measurements of blood pressure, temperature, heart rate, etc.
• Observe patients under treatment to identify progress and side-effects of medications.
• Document changes in patients’ condition and notify Physician and supervisor.
• Administer injections, prescribed medications, enema, clean and dress wounds, assist with basic care tasks.
• Collect samples for lab testing as assigned.
• Provide emotional and psychological support when needed.
• Communicate with patients’ family or friends to provide advice, comfort and discharge instructions.
