Job summary

Jacksonville Health and Rehabilitation is seeking a PRN Licensed Practical Nurse (LPN) to join our long-term care and rehabilitation team. You will provide direct nursing care, administer medications, monitor residents, and collaborate with RNs and physicians to support recovery and quality of life.

The role offers flexible scheduling (PRN), day or night shifts, and opportunities to gain experience in a compassionate, home-like setting. Certification in BLS/CPR and strong teamwork are required.

Qualifications

Responsibilities

• Administer medications and treatments as prescribed.

Position Overview

As a Licensed Practical Nurse (LPN), you will provide direct nursing care to residents while collaborating with Registered Nurses, physicians, therapists, and other healthcare professionals. Your clinical expertise and compassionate approach help ensure residents receive exceptional care every shift.

Key Responsibilities

• Administer medications and treatments as prescribed
• Monitor, assess, and document residents' health status, including vital signs
• Report changes in condition to the supervising RN or physician
• Assist residents with activities of daily living (ADLs) as needed
• Collaborate with the rehabilitation team to support recovery goals
• Maintain accurate documentation in accordance with facility policies and state/federal regulations
• Provide education and emotional support to residents and families
• Follow infection control and safety protocols to maintain a safe environment

Qualifications

• Current and valid Licensed Practical Nurse (LPN) license
• Strong clinical skills, including medication administration and wound care
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-oriented environment
• Compassionate, patient-focused approach to care
• BLS/CPR certification required
