Job Overview

We are seeking a compassionate LPN to join our team for a Private Duty assignment. The ideal candidate has a positive attitude, excellent interpersonal skills, and a current state LPN license. The role involves providing medical support to clients and ensuring their safety.

Responsibilities

1. Participate in planning and coordinating patient care with RNs and physicians, following the care plan for each patient.
2. Accurately report and document patient conditions and care, noting signs and symptoms of change.
3. Prepare clinical notes and summaries.
4. Assist patients with daily activities and teach self-care techniques.
5. Maintain a safe environment for patients.
6. Assist physicians and RNs in complex nursing situations.
7. Follow universal precautions during patient care.
8. Follow emergency procedures during catastrophic events or significant patient condition changes.

Qualifications

• Minimum 1 year of LPN experience in a home care setting.
• Current state LPN license.
• Graduate of an accredited practical nursing program.
• Proof of current CPR certification.
• Ability to work flexible hours and travel locally.
• Excellent interpersonal skills.
• Valid driver’s license and reliable transportation.
• Experience with Straight Cath is required.
