Overview

We are seeking a compassionate LPN to join our growing team! The ideal candidate possesses a can-do attitude, excellent interpersonal skills, and a current state LPN license. In this role, you will provide medical support to our clients and ensure their safety.

Responsibilities

• Participate in planning and coordinating patient care with the RN and physician, following the nursing care plan for each patient.
• Accurately report and record patient's condition and care, noting signs and symptoms indicative of change.
• Prepare clinical notes and summaries.
• Assist patients with daily activities and teach self-care techniques.
• Maintain a safe environment for patients.
• Assist physicians and RNs in complex nursing situations.
• Follow universal precautions and emergency procedures as needed.

Qualifications

• At least 1 year of experience as an LPN in a home care setting.
• Current state license as an LPN.
• Graduate of an accredited practical nursing program.
• Proof of current CPR certification.
• Ability to work flexible hours and travel locally.
• Excellent interpersonal skills.
• Valid driver’s license and reliable transportation.
