Job summary

Sayville Rehabilitation & Nursing Center in Gordon Heights, NY is seeking a licensed practical nurse (LPN) to join our growing team. The role focuses on delivering high-quality resident care, administering medications, and coordinating with the care team.

You will work under the Administrator and Director of Nursing, participate in family education, monitor residents, and supervise CNAs to ensure safe, effective care.

Qualifications

• Current LPN license for New York.
• Ability to administer medications, including IVs.
• Experience in Skilled Nursing facilities or long-term care preferred.

Responsibilities

• Provide care for residents in the Skilled Unit (SNF) and Long Term Care Unit.
• Medication administration, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcomes.
• Educate residents and families.
• Communicate with physicians, NPs, families and other health care professionals.
