Responsibilities

• Develop relationships with residents and families, placing the resident first.
• Assess and document residents' condition in the EMR, notify RN/provider as needed.
• Update care plans to reflect changes in needs and services.
• Supervise care associates and administer medications per orders.
• Participate in meetings on resident outcomes and care, respond to concerns promptly.

How you will make an impact

• Developing relationships with the residents and their families using a consistent caregiver approach supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.

• Assessing, observing, and evaluating resident’s clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary.

• Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs.

• Supervising and leading care associates

• Administering medications timely, accurately, and according to orders.

• Participating in meetings related to resident outcomes and care

• Responding to residents, family, or responsible parties in a timely and professional manner

• Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.

• Other duties as assigned by RN clinical leader

What you will need

• Licensed Practical Nurse in the Commonwealth of Virginia required. Candidates must have license number registered with the Virginia Department of Health Professions (DHP).

• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.

• Must be at least 18 years of age

• CPR certification required.

• Erickson requires all clinical care team members to maintain a validBasic Life Support (BLS) CPR certification.Employees who do not hold a current BLS CPR certification must complete the required training within 90 days from the first day of employment.

• A passion to care for seniors and be a strong resident advocate.

• Experience with electronic medical records is preferred.
