Job summary

Brighton/Suncrest Hospice is seeking an LPN to deliver compassionate hospice care across patient homes and facility settings in Indianapolis. You’ll be a key member of the care team, performing psychosocial assessments, coordinating with clinicians, and using community resources to support patients and families.

The role requires an active Indiana LPN license and at least one year of hospice or home health experience, with reliable transportation and strong communication skills.

The Licensed Practical Nurse (LPN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and Assisted Living Facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.
Must have 1 year Hospice/Home Health experience, Cn-call and Triage
  • Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice with one year of experience under license
  • Ability to work well within a team environment to collaborate daily with a team of clinicians
  • Work closely with patients on a 1-1 level but get strong support from a patient care team
  • Can work independently and on your own from a day-to-day basis with activities which include phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of state and federal rules and regulations
  • Time management skills to manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
  • Ability to learn an electronic medical records system
  • Must be willing to drive and reliable transportation, valid driver’s license, and auto insurance
