The role includes assessing residents, creating personalized service plans, and strengthening communication among physicians, staff, and families to support overall well-being and dignity.

Qualifications

• Must hold current state LPN license.
• Current CPR certification.
• Excellent communication and interpersonal skills.

Responsibilities

• Oversees medication distribution per physician orders, ensuring adherence to facility procedures and resident safety.
• Creates personalized service plans tailored to individual care needs while promoting independence and dignity.
• Fosters communication among physicians, healthcare providers and families to discuss residents’ physical, mental and social well-being.
• Contributes to quality assurance and performance improvement initiatives.
• Engages in ongoing learning through seminars, staff meetings and other learning opportunities.
