Job summary

Tender Care LLC is seeking a licensed practical nurse to support four residents with developmental disabilities in our Dover, NJ residential home. The role requires caring for clients with complex medical needs and adhering to care plans and documentation protocols.

Shifts include M-F 11pm-7am with every other weekend, offering Full-Time, Part-Time and Per Diem options; pay rate is $34-$37 per hour, with benefits such as health insurance and 401(k).

Qualifications

• Graduate from an accredited LPN program and hold a NJ LPN license.
• Minimum one year experience nursing individuals with developmental disabilities.
• Experience with Bolus feedings, G‑Tubes and Suprapubic Catheters.
• Must have NJ driver’s license and be able to commute to Dover, NJ.

Responsibilities

• Provide prescribed treatment and health care procedures as ordered by a physician.
• Monitor and document health status for each participant in the health records.
• Maintain MARs and assist with medication administration logs.
• Instruct new staff on consumer medical needs and provide quarterly in‑service training.
• Submit IRs/UIRs for medication concerns with manager support.

Job description

Tender Care LLC is a DDD service provider. We are looking for a Licensed Practical Nurse that has excellent experience with clients with developmental disabilities to support clients near Succasunna County , NJ. Our residential home serves a total of four individuals with medical needs.

Shifts Available: M-F 11pm-7am (+ every other weekend), Full-Time, Part-Time and Per Diem

Pay Rate: $34-$37/hour

Qualifications:

• Graduated from an accredited Licensed Practical Nursing program
• Current State of New Jersey LPN license
• Minimum of one year of experience providing nursing services to individuals with developmental disabilities
• Must have experience with Bolus feedings, G-Tubes, and Suprapubic Catheters
• Must have a driver’s license valid in the state of New Jersey and be able to commute to Dover, NJ

Essential Responsibilities:

• Provide prescribed treatment and health care procedures as ordered by a physician
• Advises supervisor of all concerns and/or issues related to the provision of nursing services
• Create and distribute Individual Service Plan Medical/Health Reports detailing participant’s general health, health care goals, and physician recommendations for each participant
• Document on the individual’s health status for various support people including family, guardian, staff, employment and other professional staff as necessary
• Records a detailed report of care given which reflects the therapeutic plan and specific aspects of the patient care plan on individual’s official electronic health record
• Monitor the consumer’s use of medications through Medication Administration logs
• Make referrals for needed medical services and confirming that follow-up care is pursued
• Instruct all new support staff on consumer specific medical needs
• Provide quarterly in-service training for support staff and Managers
• Monitor weight gain or loss and other health conditions of participants
• Ensure MAR’s are being followed through regular reviews and unannounced checks
• Maintain health services supplies inventory and places supply orders when the inventory is low
• Submit IR’s and UIR’s for medication errors or other medical concerns with assistance from Managers
• Provide medical training to staff as needed
• Perform other job related duties as assigned
