Job summary

Guided Steps NJ, Corp. is seeking Licensed Practical Nurses (LPNs) to provide skilled nursing care to individuals with intellectual and developmental disabilities in their homes throughout New Jersey.

Responsibilities include administering medications, G-tube care, wound and catheter care, vital signs, and coordination with RNs and caregivers. A NJ LPN license and CPR/BLS are required, plus at least two years of direct nursing experience in home or community settings.

Qualifications

• Current, unrestricted New Jersey LPN license.
• Graduation from an accredited practical nursing program.
• Experience supporting individuals with intellectual and developmental disabilities.
• Current CPR/BLS certification.
• Minimum of two years direct nursing care as an LPN.
• Experience in home care, private duty nursing, subacute rehab, long-term care, or care of medically complex individuals.
• Experience with medication administration.
• Experience with G-tube care and enteral feedings.
• Experience operating a Hoyer lift for safe transfers.
• Strong clinical observation, documentation, and communication skills.

Compensation: $35-$40 per hour, based on experience, qualifications, and clinical skills

Schedule: Flexible, based on assigned nursing needs

Position Summary

Guided Steps, an approved DDD provider, is seeking compassionate, reliable, and experienced Licensed Practical Nurses (LPNs) to provide skilled nursing care to an individual with intellectual and developmental disabilities in their home. The LPN will provide direct nursing services under the direction and supervision of the agency's Registered Nurse (RN). Each individual has unique needs and requires personalized care and support. The following outlines general responsibilities, which may vary based on each individual’s specific needs, goals, and level of support required.

Essential Responsibilities

• Administer prescribed medications and skilled nursing treatments.
• Provide G-tube care, enteral feedings, and medication administration through feeding tubes.
• Perform wound care, dressing changes, and ongoing skin monitoring.
• Provide catheter care and other prescribed nursing procedures.
• Monitor seizure-related care needs.
• Provide respiratory care, including suctioning when appropriately trained and authorized.
• Monitor vital signs, blood glucose levels, and changes in health status.
• Maintain accurate MARs and nursing documentation.
• Collaborate with caregivers, healthcare providers, and the supervising RN.

Required Qualifications

• Current, unrestricted New Jersey Licensed Practical Nurse (LPN) license.
• Graduation from an accredited practical nursing program.
• Experience supporting individuals with intellectual and developmental disabilities.
• Current CPR/BLS certification.
• Minimum of two (2) years of experience providing direct nursing care as an LPN required.
• Experience in home care, private duty nursing, subacute rehabilitation, long-term care, or care of medically complex individuals required.
• Experience with medication administration required.
• Experience with G-tube care and enteral feedings required.
• Experience with operating a Hoyer lift to assist patients with safe transfers
• Experience caring for individuals with complex medical needs.
• Strong clinical observation, documentation, and communication skills.
• Ability to work independently within the LPN scope of practice and under appropriate RN supervision.
• Reliable transportation and ability to travel to assigned service locations.
• Ability to successfully complete all agency onboarding, credentialing, and background screening requirements.

Preferred Qualifications

• Experience with seizure management, respiratory care, suctioning, wound care, and catheter care.
• Familiarity with individualized nursing care plans and medically complex populations.
• Previous experience providing services in a home or community-based setting.

Compensation, Benefits & Scheduling

• Competitive hourly compensation of $35-$40 per hour, based on experience, qualifications, and clinical skills.
• Per diem opportunities with flexible scheduling.
• Nursing assignments provided in individuals' homes throughout New Jersey.
• Travel between service locations may be required.
