Job summary

Tender Care LLC is seeking a Licensed Practical Nurse in the Succasunna, NJ area to support four residents with developmental disabilities in a residential setting. The role requires experience with Bolus feedings, G-Tubes, and Suprapubic Catheters, and entails direct patient care across multiple shifts.

Flexible Full-Time, Part-Time and Per Diem opportunities are available with shifts 7am-3pm, 3pm-11pm, and 11pm-7am, plus every other weekend.

Qualifications

• Graduate from an accredited LPN program.
• Current State of New Jersey LPN license.
• Minimum of one year of experience nursing individuals with developmental disabilities.

Responsibilities

• Provide prescribed treatment and health care procedures as ordered by a physician.
• Advise supervisor of concerns related to nursing services.
• Create and distribute Medical/Health Reports detailing health goals and physician recommendations.
• Document health status for family, guardian, staff and other professionals.
• Record care details in the electronic health record.

Job description

Tender Care LLC is a DDD service provider. We are looking for a Licensed Practical Nurse that has excellent experience with clients with developmental disabilities to support clients near Succasunna County , NJ. Our residential home serves a total of four individuals with medical needs.

Shifts Available

M-F 7am-3pm, 3pm-11pm, 11pm-7am (+ every other weekend), Full-Time, Part-Time and Per Diem

Pay Rate

$34 to $40/hour

Qualifications

• Graduated from an accredited Licensed Practical Nursing program


• Current State of New Jersey LPN license


• Minimum of one year of experience providing nursing services to individuals with developmental disabilities


• Must have experience with Bolus feedings, G-Tubes, and Suprapubic Catheters


• Must have a driver’s license valid in the state of New Jersey and be able to commute to Dover, NJ

Essential Responsibilities

• Provide prescribed treatment and health care procedures as ordered by a physician


• Advises supervisor of all concerns and/or issues related to the provision of nursing services


• Create and distribute Individual Service Plan Medical/Health Reports detailing participant’s general health, health care goals, and physician recommendations for each participant


• Document on the individual’s health status for various support people including family, guardian, staff, employment and other professional staff as necessary


• Records a detailed report of care given which reflects the therapeutic plan and specific aspects of the patient care plan on individual’s official electronic health record


• Monitor the consumer’s use of medications through Medication Administration logs


• Make referrals for needed medical services and confirming that follow-up care is pursued


• Instruct all new support staff on consumer specific medical needs


• Provide quarterly in-service training for support staff and Managers


• Monitor weight gain or loss and other health conditions of participants


• Ensure MAR’s are being followed through regular reviews and unannounced checks


• Maintain health services supplies inventory and places supply orders when the inventory is low


• Submit IR’s and UIR’s for medication errors or other medical concerns with assistance from Managers


• Provide medical training to staff as needed


• Perform other job related duties as assigned
