Job summary

Heritage Wellness & Rehabilitation in Independence, MO is seeking a compassionate Registered Nurse (RN) for day shift to provide resident-centered care in a supportive skilled nursing and rehabilitation environment.

The RN will supervise LPNs and CNAs, perform assessments, administer medications, and collaborate with physicians and the interdisciplinary team to ensure safe, high-quality outcomes for residents.

Qualifications

• Current Missouri RN license in good standing.
• Current BLS/CPR Certification.
• Experience in skilled nursing, rehabilitation, long-term care, hospital, or post-acute care preferred.
• Strong assessment, critical thinking, and leadership skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Provide skilled nursing care in accordance with physician orders and individualized care plans.
• Perform resident assessments and identify changes in condition.
• Administer medications, treatments, IV therapy, and wound care as ordered.
• Supervise and support LPNs and CNAs during the shift.
• Respond quickly to resident needs and emergencies.
• Complete accurate and timely clinical documentation.
• Communicate with physicians regarding changes in resident condition.
• Coordinate admissions, transfers, and discharges as needed.
• Educate residents and families regarding care and treatment plans.
• Follow infection prevention and infection control protocols.
• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
• Ensure compliance with CMS, state, and facility regulations.
• Foster a positive, respectful, and team-oriented work environment.
