Job summary

Heritage Wellness & Rehabilitation is seeking a compassionate Registered Nurse (RN) for day shift to provide high-quality, resident-centered care in a supportive skilled nursing and rehabilitation setting.

The RN will supervise LPNs and CNAs, perform assessments, administer medications, and coordinate with physicians and the interdisciplinary team to ensure optimal resident outcomes. Missouri licensure and BLS/CPR are required, with prior long-term care or post-acute care experience preferred.

Qualifications

• Current Missouri Registered Nurse license in good standing.
• Current BLS/CPR Certification.
• Skilled nursing, rehabilitation, long-term care, hospital, or post-acute care experience preferred.
• Strong assessment, critical thinking, and leadership skills.
• Excellent communication and teamwork abilities.
• Compassionate, dependable, and resident-focused.

Responsibilities

• Provide skilled nursing care in accordance with physician orders and individualized care plans.
• Perform resident assessments and identify changes in condition.
• Administer medications, treatments, IV therapy, and wound care as ordered.
• Supervise and support LPNs and CNAs during the shift.
• Respond quickly to resident needs and emergencies.
• Complete accurate and timely clinical documentation.
• Communicate with physicians regarding changes in resident condition.
• Coordinate admissions, transfers, and discharges as needed.
• Educate residents and families regarding care and treatment plans.
• Follow infection prevention and infection control protocols.
• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
• Ensure compliance with CMS, state, and facility regulations.
• Foster a positive, respectful, and team-oriented work environment.

Position Summary
