Hamilton Pointe Health and Rehabilitation is seeking compassionate and dedicated LPNs to join our team in Newburgh, Indiana. Night shift is available and competitive wages are offered for experience.

The LPN will provide daily care, health assessments, and documentation to support residents’ independence and well-being. Clinical supervision and ongoing training are part of the role to ensure high-quality care.

Qualifications

• Graduate of an accredited school of nursing with a valid LPN license in good standing.
• Understanding of safe and effective nursing practices within Indiana.
• Basic computer skills (Excel, Word).
• Personal integrity and tactful interaction with residents, families, visitors and staff.

Responsibilities

• Supervise, perform and document care assignments and procedures to ensure quality of care.
• Develop and implement resident care plans and maintain up-to-date nursing plans.
• Conduct daily visits to observe symptoms and responses to treatment.
• Record vital signs, intake/output, weights and diet supplements.
• Assist with orientation and training of staff to maintain skill levels.
• Report incidents and document per facility policy using the reporting system.

Responsibilities

Licensed Practical Nurse (LPN) Primary Responsibilities

The LPN is responsible for providing for the daily care, health needs, safety and comfort of the residents. Including providing professional assessments and documentation of the residents health and assisting with the management of the assigned nursing units staff.

• LPN’s will supervise, perform and provide written/verbal care assignments, routine skills and procedures to ensure the quality of care to residents
• Participate in the development and implementation of resident care plans, maintaining up to date nursing care plans, documenting progress and making recommendations for revisions to care plans according to facility policy and procedure
• LPN’s will make daily and periodic visits to observe for symptoms, reactions to medication and/or treatment
• Obtaining and documenting vital signs, intake and output, weights, diet supplements and replacements, cultures, etc.
• Assisting with the orientation and on-going training of staff to assure required skill levels are maintained for all staff
• Reporting and documenting resident and/or visitor incidents, accidents, complaints or allegations according to the facility’s policy and procedure using the designated occurrence reporting system

Qualifications

Licensed Practical Nurse (LPN) Qualifications

• A graduate of an accredited school of nursing and possess a valid LPN License in good standing according to State and Federal requirements
• Have a thorough understanding of the principles of safe effective nursing practices within the State of Indiana
• Have basic computer skills along with knowledge of such programs as Excel, Word, etc.
• Personal integrity and the ability to deal tactfully with residents, family members, visitor, government agencies and personnel
