Job summary

Homeview Health and Rehabilitation in Indiana seeks a compassionate LPN to provide daily nursing care, assessments, and documentation for residents. You will help manage assigned nursing units and support plan development and updates.

The LPN will supervise staff on shifts, observe treatment responses, obtain vital signs, and assist with training of new staff. Qualified candidates hold an LPN license and demonstrate integrity and interpersonal skills.

Qualifications

• Graduate of an accredited nursing program with a valid LPN license.
• Understand safe, effective nursing practices per Indiana regulations.
• Basic computer skills with Excel/Word.
• Personal integrity and tact with residents, families, and staff.

Responsibilities

• Supervise and perform care assignments and routine nursing tasks.
• Develop and implement resident care plans; document progress.
• Make daily visits to observe symptoms and medication responses.
• Record vital signs, intake/output, weights, and diets.
• Assist orientation and ongoing staff training.
• Report incidents per facility policy using the designated reporting system.

The LPN is responsible for providing for the daily care, health needs, safety and comfort of the residents. Including providing professional assessments and documentation of the residents health and assisting with the management of the assigned nursing units staff.

• LPN’s will supervise, perform and provide written/verbal care assignments, routine skills and procedures to ensure the quality of care to residents
• Participate in the development and implementation of resident care plans, maintaining up to date nursing care plans, documenting progress and making recommendations for revisions to care plans according to facility policy and procedure
• LPN’s will make daily and periodic visits to observe for symptoms, reactions to medication and/or treatment
• Obtaining and documenting vital signs, intake and output, weights, diet supplements and replacements, cultures, etc.
• Assisting with the orientation and on-going training of staff to assure required skill levels are maintained for all staff
• Reporting and documenting resident and/or visitor incidents, accidents, complaints or allegations according to the facility’s policy and procedure using the designated occurrence reporting system

Licensed Practical Nurse (LPN) Qualifications

• A graduate of an accredited school of nursing and possess a valid LPN License in good standing according to State and Federal requirements
• Have a thorough understanding of the principles of safe effective nursing practices within the State of Indiana
• Have basic computer skills along with knowledge of such programs as Excel, Word, etc.
