Senior Lifestyle Bayshore Memory Care in Naples, FL is seeking a Licensed Practical Nurse (LPN) for part-time, PRN coverage in Memory Care. The role supports residents with licensed nursing services, medication administration, and care planning within a collaborative team.

The LPN must have an active in-state license and CPR certification, with 1+ year of experience in senior care. Vaccination and DailyPay benefits are offered as part of a comprehensive benefits package.

Responsibilities

• Provide licensed nursing care to residents per Senior Lifestyle guidelines.
• Administer medications in accordance with Federal & State regulations.
• Assist with admissions, assessments and development of care or service plans.
• Monitor health status and communicate changes to physicians and families.
• Maintain accurate clinical documentation and track wounds/infection control as directed.
• Respond promptly to emergencies and coordinate with the team.

Do you have HEART? This position is responsible for providing licensed nursing services to residents, following the guidelines provided by Senior Lifestyle Corporation.

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.


• May be responsible for the admissions of new residents to include all admission assessments and documentation.


• Responsible for completing acute and long‑term care plans or service plans.


• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.


• Provide general consultation regarding health-related concerns within the community.


• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.


• Respond quickly to all emergencies.


• Maintain close communication with all departments and staff members.


• Provide educational meetings for residents and staff.


• Understand role in the Safety & Disaster Plan.


• Maintain confidentiality of residents’ and prospective residents’ information.


• May be responsible for discharges, assessments, and clinical documentation as needed.

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Type of Experience: Senior care or related field
Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations.
