Job summary

AdventHealth Palm Coast is seeking an experienced Licensed Practical Nurse (LPN) for hospice-oriented patient care at the Stuart Meyer Inpatient Unit and surrounding facilities. You will follow care plans, administer medications, and support patients and families facing end-of-life situations.

The role emphasizes compassionate bedside caregiving, accurate documentation, and collaboration with an interdisciplinary team to uphold AdventHealth’s standards and the patient’s comfort throughout care

Qualifications

• Knowledge of hospice care principles and patient comfort measures.
• Ability to follow care plans and safety protocols.
• Experience with electronic medical records (EMR) systems.
• LPN license and current CPR/BLS certification.

Responsibilities

• Provide direct patient care to ensure comfort and meet changing needs.
• Administer medications and treatments as prescribed.
• Document patient care activities in compliance with Medicare regulations.
• Participate in interdisciplinary team meetings and care planning.
• Maintain accurate and timely patient records.
