Job summary

AdventHealth in Daytona Beach, FL seeks a licensed Practical Nurse to provide home-health patient care across South Volusia County. You will administer medications, perform basic nursing procedures, educate patients and families, and coordinate with RNs to ensure quality care.

Full-time day shift with a schedule that includes weekends as needed. Requires LPN licensure, good communication, and ability to work independently in patient homes.

Qualifications

• High school diploma or equivalent required.
• Technical or vocational training in nursing required.
• LPN license or certification required.

Responsibilities

• Provides patient education and administers medications as ordered.
• Assists RN with wound care and ostomy care per protocol.
• Performs patient assessments and documents vital signs.
• Participates in weekend/after-hours coverage as needed.
• Maintains accurate clinical records in the patient chart.

Job Description

Sign On Bonus For eligible applicants, up to $2500

Schedule: Full Time

Shift: Monday - Friday 8am – 5pm, flexible. Every 3rd weekend. No Call!

Location: Home Visits covering South Volusia County. Oak Hill, South Daytona, Daytona, Daytona Beach Shores, Ponce Inlet, Port Orange, NSB, Edgewater

• Develops professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations.
• Participates in the review process and the agency performance improvement program.
• Provides patient education, medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician.
• Displays flexibility in accepting assignment changes and participates in weekend/after-hours coverage and call rotation.
• Reports pertinent observations to the attending physician and RN, and implements changes in the patient care plan as directed.
• Communicates changes to the appropriate members of the home health care team.
• Demonstrates effective communication skills with all team members, patients, and families.
• Maintains clinical record information in accordance with established standards and within established time frames.
• Provides direct patient care responsibilities for assigned patients in an efficient, organized manner under the direction of the RN.
• Performs treatments within their scope of training and administers medications to assigned patients.
• Assesses patients during clinical check-in, documents pertinent medical information and vital signs, and assesses for signs of abuse or neglect using age-specific guidelines.
• Administers treatments, medications, and follow-up assessments ordered by the provider, verifying as needed, and responds appropriately in clinical emergencies and triage situations.
• Performs diagnostic and occupational medicine testing, including EKG, spirometry, audiometry, titmus, and pulse oximetry, and complete forensic testing collections in accordance with established guidelines.
• Collects data specific to the age of the patient and contributes to their plan of care.
• Receives, verifies, and implements physician orders, ensuring orders taken by the LPN are co-signed by an RN.
• Other duties as assigned.
