Job summary

HomeFirst Home Healthcare is seeking a Licensed Practical Nurse (LPN) for full-time home health care in Sumner County, Tennessee. You will provide one-on-one patient care, teach families, and follow the established plan of care in patients' homes.

The role requires an accredited LPN program graduate with an active LPN license, CPR certification, and strong communication, organizational, and collaborative skills. Valid license, transportation, and insurance are a must.

Qualifications

• Graduate of an accredited Licensed Practical Nurse educational program.
• Current Licensed Practical Nurse (LPN) license within the state of practice.
• Current CPR certification.
• Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills.
• Reliable transportation, valid driver's license and current automobile insurance.

Responsibilities

• As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting.
• The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care.
• The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.
