Job summary

HomeFirst Home Healthcare is seeking a Licensed Practical Nurse (LPN) to provide one-on-one in-home care in Grundy and Franklin counties. The LPN administers skilled nursing care, follows the plan of care, and documents activities promptly while coordinating with the interdisciplinary team.

This PRN role offers competitive per-visit pay of $40-$45, mileage reimbursement, benefits, and an employee referral program.

Qualifications

• Graduate of an accredited LPN program.
• Current LPN license in Tennessee.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Strong organizational, collaboration and time-management abilities.
• Reliable transportation and valid driver’s license with insurance.

Responsibilities

• Provide one-on-one patient care in the home setting and family teaching.

Overview

As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting. The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.

Requirements

• Graduate of an accredited Licensed Practical Nurse educational program
• Current Licensed Practical Nurse (LPN) license within the state of practice
• Current CPR certification
• Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
• Strong initiative, follow through, process orientation, attention-to-detail
• Reliable transportation, valid driver’s license and current automobile insurance
