Job summary

Elite Home Health in Florida is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under RN direction. The role emphasizes compassionate, patient-centered care and adherence to regulatory requirements.

This position requires graduation from an accredited Practical Nursing program and a current Florida LPN license with CPR certification. At least 1 year of nursing experience is preferred, with home health/hospice experience a plus;

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Florida LPN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to meet all required background and compliance requirements.

Responsibilities

• Provide skilled nursing care to patients in the home setting
• Administer medications and treatments within LPN scope of practice
• Perform wound care, dressing changes, catheter care, and other skilled nursing tasks
• Monitor patient condition and report changes to the supervising RN
• Reinforce patient and caregiver education
• Follow established plans of care and physician orders
• Complete accurate and timely documentation in the EMR
• Communicate with RNs, physicians, and other members of the care team
• Participate in required training, meetings, and in-services
• Provide care in accordance with Medicare requirements, state regulations, and agency policies

Position Summary
