Job summary

Elite Home Health is hiring a full-time Licensed Practical Nurse (LPN) to provide skilled nursing care in patients’ homes under RN direction. Coverage area includes Clermont and surrounding areas, with Medicare-certified home health services across Florida.

The ideal candidate holds an active Florida LPN license, CPR certification, and at least 1 year of nursing experience, with home health, hospice, or acute care background preferred.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Florida LPN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.

Responsibilities

• Provide skilled nursing care to patients in the home setting.
• Administer medications and treatments within LPN scope of practice.
• Perform wound care, dressing changes, catheter care, and other skilled nursing tasks.
• Monitor patient condition and report changes to the supervising RN.
• Reinforce patient and caregiver education.
• Follow established plans of care and physician orders.
• Complete accurate and timely documentation in the EMR.
• Communicate with RNs, physicians, and other members of the care team.
• Participate in required training, meetings, and in-services.
• Provide care in accordance with Medicare requirements, state regulations, and agency policies.

Position Summary

The LPN provides skilled nursing services in accordance with physician orders, established care plans, RN direction, agency policies, and applicable state and federal regulations.
