Elite Home Health is a Medicare-certified home health agency serving Florida. We seek a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under RN direction.

You must be a graduate of an accredited Practical Nursing program with an active Florida LPN license and CPR certification. The role emphasizes compassionate, patient-centered care and compliance with state and federal regulations.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Florida LPN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in the home setting.
• Administer medications and treatments within LPN scope of practice.
• Perform wound care, dressing changes, catheter care, and other skilled tasks.
• Monitor patient condition and report changes to the supervising RN.
• Reinforce patient and caregiver education.
• Follow plans of care and physician orders.
• Document accurately and timely in the EMR.
• Communicate with RNs, physicians and care team.
• Participate in trainings and in-services.
• Provide care in accordance with Medicare and state regulations.

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the direction of a Registered Nurse (RN).

Position Summary

The LPN provides skilled nursing services in accordance with physician orders, established care plans, RN direction, agency policies, and applicable state and federal regulations.

Responsibilities

• Provide skilled nursing care to patients in the home setting
• Administer medications and treatments within LPN scope of practice
• Perform wound care, dressing changes, catheter care, and other skilled nursing tasks
• Monitor patient condition and report changes to the supervising RN
• Reinforce patient and caregiver education

• Complete accurate and timely documentation in the EMR
• Communicate with RNs, physicians, and other members of the care team
• Participate in required training, meetings, and in-services
