Job summary

Elite Home Health in Florida is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under RN supervision. You will administer medications, perform wound care, and reinforce education for patients and caregivers.

The ideal candidate holds an accredited LPN program diploma, a valid Florida license, CPR certification, and reliable transportation. Home health experience is preferred, and you must meet state and federal regulations.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Florida LPN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to meet all required background and compliance requirements.

Responsibilities

• Provide skilled nursing care to patients in the home setting.
• Administer medications and treatments within LPN scope of practice.
• Perform wound care, dressing changes, catheter care, and other skilled nursing tasks.
• Monitor patient condition and report changes to the supervising RN.
• Reinforce patient and caregiver education.
• Follow established plans of care and physician orders.
• Complete accurate and timely documentation in the EMR.
• Communicate with RNs, physicians, and other members of the care team.
• Participate in required training, meetings, and in-services.
• Provide care in accordance with Medicare requirements, state regulations, and agency policies.

Position Summary
