• Hourly Range: $24.00 - $31.00

Responsibilities

• Verify plan of care with RN prior to visits
• Collaborate with physicians and RNs; notify them of patient condition changes
• Teach self-care techniques to patients and caregivers
• Check vital signs and perform medication reconciliation
• Perform specialized procedures as ordered
• Prepare equipment using aseptic techniques
• Complete documentation accurately and timely

Qualifications

• Minimum one year of clinical LPN experience
• Current LPN license for the state of practice
• CPR certification
• Valid driver’s license, reliable transportation, and liability insurance
