Job summary

Elite Home Health, a Medicare-certified home health agency serving Florida communities, seeks a dedicated LPN for PRN coverage in Ocala and surrounding areas. The LPN will provide skilled nursing care in patients' homes under the direction of an RN, following established care plans and Medicare requirements.

Ideal candidates are graduates of an accredited practical nursing program with an active Florida LPN license, CPR, reliable transportation, and the ability to work independently.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Florida LPN license in good standing.
• Minimum 1 year nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to meet background/compliance requirements.

Responsibilities

• Provide skilled nursing care to patients in the home setting.
• Administer medications and treatments within LPN scope of practice.
• Perform wound care, dressing changes, and catheter care.
• Monitor patient condition and report changes to the supervising RN.
• Reinforce patient and caregiver education.
• Follow established plans of care and physician orders.
• Complete accurate and timely documentation in the EMR.
• Communicate with RNs, physicians, and other members of the care team.
• Participate in required training, meetings, and in-services.
• Provide care in accordance with Medicare requirements, state regulations, and agency policies.

Position Summary

The LPN provides skilled nursing services in accordance with physician orders, established care plans, RN direction, agency policies, and applicable state and federal regulations.
