BAYADA Home Health Care

is looking for a compassionate and dedicated Licensed Practical Nurse, LPN to join our home health visits team. This office works with adult and senior clients on a per visit basis in homes throughout Union County, Somerset County, and Middlesex County.

This is a full-time opportunity. One year of prior clinical experience as a licensed Practical Nurse is required. As a home care nurse, you will be an integral member of a multi-disciplinary healthcare team that provides skilled nursing and rehabilitative care, enabling clients to receive the medical attention they need to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

• Conduct home visits to clients in designated areas
• Administer medications, perform wound care, treatments, and procedures
• Monitor client conditions and report changes to the Clinical and Client Services Managers
• Follow doctors' orders accurately and document them properly
• Perform client assessments as needed
• Manage cases and coordinate care
• Document observations, interventions, and evaluations using electronic medical records on a tablet

Qualifications:

• Current NJ Licensed Practical Nurse license
• Minimum of one year of recent, verifiable LPN experience
• Graduation from an accredited nursing program
• Home care experience preferred but not required
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Proficient computer skills; EMR experience preferred
• Ability to travel to assigned cases
