Job summary

Fresh Perspective Home Care in Kalamazoo, MI is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one in-home care. Schedule: Monday, Wednesday and Friday, 9:00 AM to 1:00 PM, with duties including medication administration, care plan implementation, and transportation assistance.

You will document changes, communicate with supervisors, and uphold client confidentiality while delivering high-quality, patient-centered care in a home setting.

Qualifications

• Active LPN license in good standing.
• Ability to lift, transfer, and assist up to 40 lbs.
• Valid driver's license and reliable transportation.
• Ability to work independently in a home setting.

Responsibilities

• Provide direct nursing care in accordance with physician orders and the care plan.
• Administer medications and treatments.
• Monitor and document changes in client condition.
• Communicate with supervisor regarding client needs and concerns; complete accurate and timely documentation.
• Provide transportation assistance as needed.
