Job summary

Award Care is seeking a qualified Licensed Practical Nurse (LPN) to join our family and provide direct patient care in a skilled nursing/rehabilitation center. Newly licensed nurses are welcome to apply, with opportunities to learn and grow in a supportive team.

We offer FT Day shifts (7a-7p) with a bi-weekly $225 bonus and 12-hour shifts that give you four days off. Competitive pay and overtime may apply. Equal opportunity employer.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• You must be qualified, compassionate, and dedicated to a job well done.

Responsibilities

• Provide direct patient care to residents in a skilled nursing/rehabilitation setting.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services according to individualized treatment plans.
• Document and evaluate residents' medical status throughout treatment.

Major Responsibilities - Licensed Practical Nurse

The Licensed Practical Nurse (LPN) care giver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:

• Assist in the implementation of individualized treatment plans for each assigned resident.

• Supervise CNAs and participate in their evaluation.

• Oversee Falls Program and Pressure Sore Program.

• Provide nursing services.

• Conduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment.

• Other duties as assigned.

Qualifications - Licensed Practical Nurse
