Position Overview

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, administering treatments, and collaborating with the healthcare team to achieve positive patient outcomes.

Essential Functions
  • Provides direct patient care as defined by the State Nurse Practice Act, including end-of-life care duties.
  • Implements nursing care based on comprehensive assessments and care plans.
  • Documents patient services accurately and promptly.
  • Educates patients and families about diagnosis and care plans.
  • Coordinates hospice services, reporting needs to the interdisciplinary team.
  • Prepares and uses equipment effectively, adhering to aseptic techniques.
  • Manages pain and symptoms, evaluating treatment responses.
  • Participates in professional development and quality improvement activities.
Work Environment & Physical Requirements

This role involves physical activities such as standing, lifting, bending, and twisting. A valid driver’s license and a well-maintained insured vehicle are required. The work environment may include weather-related conditions and travel.

Minimum Education & Experience
  • Licensed Practical Nurse licensed and in good standing with the Wisconsin State Board of Nursing from an accredited program.
Preferred Qualifications
  • 1-2 years of nursing experience in acute care and at least 1 year in home health or hospice.
  • Interest in hospice philosophy and comfort care.
  • Ability to work collaboratively with multidisciplinary teams.
